The following are steps you will need to do to apply for a Dignity Health Community Grant.

• Step One: Identify your local community grants representative Community Grants Program Area Representative
• Step Two: Select hospital service area 2013 Community Grants Brochure
• Step Three: 2013 Letter of Intent (LOI)
• Step Four: 2013 Community Grant Proposal

Other Reports: Accountability Report 2011 (FY2012) due March 31, 2013; Accountability Report 2012 (FY2013) due March 31, 2014. Submit electronic report to pablo.bravo@dignityhealth.org and elizabeth.ramas@dignityhealth.org
