Care for Chronic Conditions


For those living with chronic disease, support services are often the missing link between the physician’s office, the hospital, and the home. Dignity Health has taken a bold and strategic approach to chronic disease management, developing programs at our medical facilities that address chronic disease in the community, with a specific emphasis on those who are poor and vulnerable.

Many Dignity Health hospitals offer disease management programs, and all provide disease-specific discharge education for patients as a standard of quality care. Chronic disease self-management programs ultimately result in reduced health care expenditures, improved health outcomes and fewer unnecessary hospital visits.

Patients who participate in our chronic disease self-management programs are also able to better maintain many of their health and behavioral improvements over time, leading to significant, measurable progress in patient outcomes and quality of life.

Palliative care is end-of-life care, a comprehensive, interdisciplinary approach to supporting patients with terminal illnesses and their families. Because even when we cannot cure, it is still our mission to care.
