Congestive Heart Active Management Program

Dignity Health - St. Rose Dominican offers a comprehensive program to assist those who have a condition known as heart failure. This condition simply means your heart has weakened, which can cause shortness of breath and reduced stamina. The CHAMP® program involves you in your own recovery. CHAMP® RNs, through regular phone interaction and education, will help you and your family better understand and manage this condition. 

Who makes up the CHAMP® team?
Cardiologists, primary care physicians, registered nurses (RN, BSN), clinical pharmacists, registered dietitians and other health care professionals.

How do I get into the program? 
You are referred to the program by your physician.

How will I benefit from CHAMP®? 
CHAMP® provides a vital link to the medical world after you leave the hospital. It gives you and your family the knowledge and support you need to help you maintain the highest quality of life and reduce your risk of being readmitted to the hospital.

How does it work?
 
After you leave the hospital, a CHAMP® RN will call you at home to check on how you feel, counsel you on diet and fluid limitations, check on your weight and ask how you are managing your medications. Call frequency is established based on your individual needs as you learn more about managing your heart failure. CHAMP® RNs monitor your symptoms or complications. Working with your doctor (who continues to be in charge of your care), RNs provide you with recommendations on diet changes, medication modifications or if you need an office visit.

CHAMP® has earned the Gold Seal of Approval Certification for disease-specific care from the Joint Commission on Accreditation of Healthcare Organizations.
