Important Information About Your Medi-Cal Coverage

If you currently receive health insurance through Medi-Cal or Children’s Health Insurance Program (CHIP), you must provide the county with your updated information so you can continue receiving important information about your Medi-Cal coverage.

Why is this happening?
During the COVID-19 pandemic, all Medi-Cal or CHIP recipients were continuously enrolled in health insurance coverage without needing to reapply for benefits. Due to a recent federal law, continuous Medi-Cal enrollment ended on March 31, 2023, so you need to take steps to ensure your health insurance coverage will continue if you are eligible.

Cardiac Rehab

Cardiac Rehabilitation is a program of cardiac-specific exercise and education designed to assist you in resuming and maintaining a healthy lifestyle.

Our specially trained staff work with physicians to develop an individualized plan of Cardiac Rehabilitation for each patient. All patients with a cardiac diagnosis are candidates for outpatient Cardiac Rehabilitation.

Exceptional Hometown Health Care
