Please bring the following to your appointment

Arrival instructions

Please arrive 15-30 minutes early to allow time for parking and filling out any paperwork. If applicable, please remember to bring your parking ticket for validation. If your provider’s office is located on the Dignity Health St. Joseph’s campus, you may park in the 6th Avenue Garage; there is also valet parking available for a fee.

Insurance prior authorization and referrals

Please note that many insurance plans require prior authorization before a patient can be seen by a specialist. Failure to receive prior authorization/referral when required may result in the need to reschedule your appointment to a later date. Please contact your insurance company for more information or to see if a prior authorization is required.

Telephone communications

For a life-threatening medical emergency, please call 9-1-1 immediately to obtain immediate access to local emergency medical services.
Our telephones are answered during posted business hours. Our team is prepared to handle your general questions and will direct questions as needed. After hours and on weekends or holidays, our answering service is here to assist you. If necessary, they will contact the on-call provider.​

Dignity Health Patient Portal

To access you health records, communicate with your doctor, and more log into My Portal Patient Portal. If you do not have a My Portal account, please provide your email address to the office staff when you check in to your next appointment!​

Written communications

Please do not fax or mail any time-sensitive communication or urgent medical advice questions to the office. Doing so may result in a delay in response to your time-sensitive communication.​

Medical questions

Please call the office directly to discuss any concerns about your medical condition or if you have questions about your medications or laboratory results. If your call is of an urgent nature, please call the main number and inform the staff that you need urgent assistance. Remember, for a true medical emergency do not wait for a return call from the office. Instead, call 9-1-1 to obtain immediate access to local emergency medical services.

Appointment reminder

You will receive an automated telephone call or text message to remind you of your appointments. For a telephone reminder, please listen carefully and select one of the following message options:

  • Please press 1 to confirm your appointment.
  • Please press 2 to repeat this message.
  • Please press 3 to cancel or reschedule your appointment.
  • Please press 4 to hear directions to our office.
  • If you do not wish to receive an appointment reminder, please contact our office directly.​

Cancellation of scheduled appointments

Should you need to cancel or reschedule an appointment, we kindly ask that you notify us as soon as possible--no later than 24 hours prior to the appointment--so we can reschedule you in a timely manner and offer the open slot to another patient. If you are late for your appointment, you may have to be rescheduled. Multiple cancellations or failure to show will impact our ability to care for you.​

Pharmacy refills

For refills on medications prescribed by a doctor from this office, please ask your pharmacy to fax a refill request to our office directly. It is important that you do not run out of your prescribed medications. Please plan ahead for refills and allow three business days for faxed refill requests to be processed and returned to the pharmacy. Keep in mind that prescriptions for certain controlled substances require a hard copy prescription that cannot be called in or faxed. It is the policy of our doctors not to refill controlled substance prescriptions for our practice partners; therefore, you must obtain these prescriptions from your prescribing provider.

Test ordering and results

Your doctor may order diagnostic tests as part of your evaluation and care. Some insurance companies require prior authorization and approval before your test can be scheduled. Your doctor will submit orders for any required tests; our Referrals and Authorization team will send required documentation to obtain authorization from your insurance company prior to scheduling. Questions about this process can be directed to (602) 406-3464(DHMG). Upon receipt, office staff will arrange the delivery of results; most results can be expected within three weeks of testing and are available on the patient portal.
