Mercy Home Health enables patients to recover from illness or surgery at home rather than in a rehabilitation unit. Our registered nurses also provide care for patients who are terminally ill, injured, frail, or meet other criteria determined by their doctor. We also offer specialty programs for palliative care, heart failure, wound care, and chronic obstructive pulmonary disease patients. All of our professional staff is licensed, and our program has received outstanding recognition from The Joint Commission.
Our experienced care team of nurses, therapists, aides, and social workers, understand the importance of home health in the care continuum, and they have the clinical skills to meet home care patient needs. Partnering closely with doctors and caregivers, they work to enhance patients' health, helping them attain the highest levels of independence and quality of life.

If you are in the hospital, speak to your nurse or discharge planner about selecting Mercy Home Health for your home care services. If you are in the community, speak to your primary care doctor. We accept doctor referrals.
