Living with Diabetes


Almost 26 million children and adults in the United States live with the lifelong disease of diabetes. When you have diabetes, you either don't produce enough or don't correctly use insulin. Insulin is a hormone that converts sugar, starches and other foods into energy, and when it is not functioning properly it causes your blood sugar levels to be too high. With proper treatment, those with diabetes can live an active and full life.

Diabetes Education Program

We provide quality outpatient education and support for diabetes self-management. Our certified diabetes educators provide instruction and support on Type 1 and Type 2 diabetes, gestational diabetes and prediabetes. Spanish interpretation is available for most services and all classes.

Group classes

Our regularly scheduled group classes include:

  • Pre-diabetes. Learn what pre-diabetes is and how it is diagnosed. We discuss weight management, healthy eating and exercise for those at risk of developing the onset of Type 2 diabetes.
  • Tips and Tools for a Sweet Life™ is an evidenced-based, self-management program designed to teach people how to successfully manage type 1 or type 2 diabetes over the course of 6-weeks.  Participants receive world-class education that is patient-centered, interactive, and clinically effective.
  • Gestational diabetes (diabetes during pregnancy). Most women who have gestational diabetes have no symptoms. It is diagnosed through an oral glucose test. So, if you are pregnant, or considering pregnancy, it is a good idea to learn the risks, understand the complications and treatment options.    

Free Support Groups

  • “A Sweet Life” Outreach Group with guest speakers held on the first Tuesday of alternating months at 6 p.m. Call for current schedule and topics.
  • Pumpers Support Group. This group is for anyone with diabetes who wears an insulin pump or is considering a pump. Group meets the third Tuesday of each month at 7 p.m. 

    No registration required.

Individual Consultation

We offer diabetes consultation in addition to group education. If you need help with areas related to diabetes management and self-care including the following, please contact our office:

  • Insulin initiation and management
  • Blood glucose meter training and monitoring
  • Insulin pump training and continuous glucose monitoring
  • Healthy eating and active living
  • Reducing risks and complications
  • Monthly educational outreach groups
  • Pediatric diabetes
  • Diabetes refresher with medications review, healthy eating, and blood sugar results monitoring

If you are interested in receiving diabetes education, consultation or support, please call us at 480.728.3535.  Classes are offered at various times of day throughout the week. Enrollment for prediabetes class ONLY is available online. Learn more about our other classes, consultation services, and free outreach groups.

A physician’s referral is required at the time of scheduling.

Physician’s Referral

If you would like to work with an educator to improve your diabetes control,   please talk to your doctor about participating in these classes. Ask your doctor’s office to download the order form. If you would like, our front office can contact your provider on your behalf.

Upon receipt of all materials, we will contact you to discuss our classes and services. Your physician will receive progress notes as you progress through the program. Our comprehensive Diabetes Self-Management training is covered by most insurance plans, including Medicare Part B.

Health Care Providers 

Center for Diabetes Management is staffed with Certified Diabetes Educators who can help improve outcomes for patients with diabetes. (average drop in A1c is 1.6% six months after patients complete all classes.) If you would like to refer patients to any of our classes, please download the order form, attach patient labs, and fax to 602.798.0775. You will receive progress notes as your patient progresses through the program.

Billing Questions

Diabetes Self- Management training is a covered benefit on most insurance plans, including Medicare Part B. It is billed as an outpatient service of Mercy Gilbert Medical Center.

Directions and Parking

Center for Diabetes Management is located in Rome Towers, 1760 E. Pecos Road, #235, Gilbert 85295 (East of Pecos Road and Val Vista Drive).

From the 202 Highway San Tan east or west, exit Val Vista Drive and turn north. From Val Vista Drive you will turn east (right) to Pecos Road. Rome Towers will be on your left hand side. Turn north (left) onto Rome to enter the parking garage behind the building and proceed to the second level to park.

Take the bridge on the second level into the office building. We are located on the second floor in Suite 235.

Center for Diabetes Management provides comprehensive education and support for individuals and families within our community affected by diabetes and pre-diabetes. Our mission is to enhance diabetes self -management skills, allow people to be confident in their ability to manage their disease, improve their quality of life, and prevent costly and life altering complications of diabetes. Our staff consists of Certified Diabetes Educators who are registered nurses and registered dietitians, specially trained in the management of diabetes. We are committed to helping each person that walks through our door, wherever they are in their journey with diabetes, to improve their health and quality of life.
