Medical Records

Accessing your medical records is easy using our online patient center. You can also download, fax or mail in your request for medical records.

Access Your Records Online

We now offer an Online Patient Center for our hospital patients. You or a designated family member can view your health records, download them or send them to your personal doctor. For help enrolling, just ask us.

Visit My Care - Dignity Health and select Northridge Hospital Medical Center from the drop down menu to log on and access your hospital patient records.

Requesting Records via Fax/Mail

To start your request, simply go to this link - Patient Request Form. This is a form you can print and fill out.  It can also be filled out on your device, saved, and printed for signing.  Once signed, fax or mail it to the attention of the Health Information Management department at the fax number or address below. Please be sure to sign the form; unsigned requests cannot be processed.

We will mail your medical records to the address specified on the authorization form. An invoice may accompany medical records requests.
