Subacute — Complete Neurological Care

As part of the rehabilitation program, our residents’ health care plan may include: 

• Physical therapy
• Speech therapy
• Occupational therapy
• Life activities (activities of daily living)
• Community and interpersonal socialization
• Educational programming which can include school participation (pediatric patients only)
• Other developmental activities 
• Comfortable and convenient access to all other services offered at the hospital, including:
  • Registered nurses, licensed vocational nurses, and certified nursing assistants

  • Laboratory, radiology, and diagnostic services

At Community Hospital of San Bernardino, our teams of subacute specialty physicians, highly trained nurses, case managers and therapists offer excellent patient care and compassion. 

Patients who require dialysis treatments will now have on-site access to Community Hospital's new dialysis unit.

Acute care is the immediate, short-term intensive care given when a patient has a critical/debilitating injury or illness with the goal to return to their daily life after a short term stay. Subacute care is for patients who continue to need a high level of care that is still very specialized but less intensive than acute care. In acute and subacute care, our nursing staff are required to have special training and, in some instances, special licensing in order to meet the needs of the patients. 

While the goal is to help patients regain as much mobility and independence as possible, long-term care is a common need for subacute patients. Efforts to ensure comfort and quality of life are woven into the program. This includes social services support to help you navigate insurance and accommodations for visiting, where possible. At Community Hospital of San Bernardino, we care about you and your loved one’s comfort and wellbeing.

We offer two subacute units for those who have specialized care: One unit is for adults and the second is a pediatric unit for children under the age of 21 years. The proximity of our care units to the hospital provides convenience for our patients and their families when medical care outside of standard subacute care is needed.
