Home Health, Hospice, Private Duty, Geriatric Care Management
Pathways, a not-for-profit affiliate of Sequoia Hospital and Dignity Health, has been at the forefront of care at home since 1977. Pathways helps prevent hundreds of hospitalizations a year by providing nursing care, rehabilitation therapy, and other services needed to help you meet your health care goals.
- Home Health. Pathways provides hands-on care, as well as teaching the patient and caregivers how to manage care independently at home.
- Hospice. Pathways delivers holistic medical, nursing, spiritual, emotional, and personal care to people approaching the end of life and their families.
- Private Duty. Pathways helps older adults remain independent, helping with activities such as bathing, incontinence care, laundry, errands, eating, and walking.
- Geriatric Care Management. Pathways care managers oversee older adults when family is unable, arranging services as diverse as meal delivery, medical visits, or home repairs.
