Home Health

Patients benefit from the calm and familiarity of home and the expert care provided by the Mercy Home Health staff – together this can make a big difference in your recovery.

Our Home Health program enables patients to recover from illness or surgery at home rather than in a rehabilitation facility or skilled nursing unit. Our registered nurses also provide care for patients who are terminally ill, injured, frail, or meet other criteria determined by their doctor. 

We also offer specialty programs for palliative care, heart failure, wound care, and chronic obstructive pulmonary disease patients. All of our professional staff is licensed, and our program has received outstanding recognition from The Joint Commission.

Our Expert Staff

Our experienced care team of nurses, therapists, aides, and social workers understand the importance of home health in the care continuum, and they have the clinical skills to meet home care patient needs. Partnering closely with doctors and caregivers, they work to enhance patient's health, helping them attain the highest levels of independence and quality of life.

Eligibility

  • Patient is under the care of a local doctor
  • Must have a skilled need
  • Must meet Medicare’s definition of “home bound”

How to Arrange for Services

If you are in the hospital, speak to your nurse or discharge planner about selecting our Home Health program for your home care services. If you are in the community, speak to your primary care doctor. We accept doctor referrals. For more information about our Home Health program, call us at (530) 529-7187 Monday through Friday from 8 a.m. to 5 p.m. Our answering service will be able to assist you after hours.
