Diabetes Navigator

Newly diagnosed or need help managing diabetes? Our Diabetes Navigator is ready to help find the services best for you. Whether that be one of our class offerings, or another community based service, if the support you are needing is available, we are happy to help you find it.  Consider the Diabetes Navigator as your friend in your diabetes journey.  Support is just a phone call away, (209) 944-8355.

All classes are open to the community. We encourage you to visit our community health program page for flyers on all of our current programs.

Power Hour

Empowering individuals with knowledge and support is the goal of this no cost program available to any San Joaquin County resident. Participants will gain knowledge, skills, and confidence from a Certified Diabetes Care and Education Specialist (CDCES) to improve health outcomes.  NO INSURANCE or REFERRAL REQUIRED.

Every 1st Wednesday of the month, 12 pm to 1 pm

Sugar Fix Support Group

A diabetes diagnosis is full of highs and lows in both blood sugar levels and in frustrations. If diabetes has you feeling down and not your best self, you’re not alone. Join this monthly group to hear how others manage some of the same issues you are experiencing.

Meetings are In-Person

Every 3rd Wednesday of the month

from 3:30 - 4:30 pm

Diabetes Exercise Class

Exercise in a supervised safe environment with an individualized program. Regular exercise has been shown to help control blood sugar as well as lower the risk of diabetes related health complications such as heart disease and stroke.

    Classes will be held twice a week at St. Joseph’s Medical Center

    Each participant will have an individual evaluation and personalized program

    Classes are designed to build strength, and increase endurance and flexibility

    Classes supervised closely by qualified exercise trainers.

Diabetes Empowerment Education Program (DEEP)

Do you or someone you care for have diabetes or prediabetes? Join us for the FREE Diabetes Empowerment Education Program™ (DEEP). In this six session workshop you will learn about diabetes and you’ll also be equipped to make lifestyle changes to improve your quality of life. 

Workshop Details:

  • Six, 2-hour weekly sessions covering important topics for healthier living
  • Sessions are interactive and involve demonstrations, games, and other activities for fun group learning
  • Weekly participation raffles, and prizes for all who complete the series
  • No cost to participate
