
Grant Recipients Announced
Chandler Regional and Mercy Gilbert Medical Centers, Dignity Health Members, announced the recipients of the 2013-14 Community Grants program. This year, organizations were required to come together to create a Community of Care (minimum of three) and submit collaborative applications with focus on*:
- Transitional care after discharge from Chandler Regional or Mercy Gilbert
- Continuum of care after discharge from Dignity Health emergency room
- Provide community-based intervention, prevention, and education and/or management measures for diabetes, heart disease, congestive heart failure, asthma, chronic obstructive pulmonary disease, infection/sepsis, mental health and obesity.
The 2013-14 Community of Care recipients are:
East Valley Holistic Family Care: $100,000
Chandler Education Foundation - $18,000
Hope Community Health Center - $43,000
Southwest Behavioral Health - $39,000
East Valley I-Help Coalition: $99,999
Chandler Christian Community Center - $33,333
Lutheran Social Services of the Southwest - $33,333
Tempe Community in Action - $33,333
Senior Community Wellness Coalition: $82,276
About Care - $22,000
Valley of the Sun YMCA/Ahwatukee - $18,000
Neighbors Who Care - $42,276
The objective of the Dignity Health Community Grants Program is to award grants to nonprofit organizations whose proposal responds to the Dignity Health East Valley's Strategic priorities identified in the community health needs assessment and the community benefit plan.
“Our goal is to continue to improve community health through better access, prevention and education,” said Kathleen Dowler, director of Chandler Regional and Mercy Gilbert’s community integration department. “These awardees have identified unmet health-related needs throughout our community and we are pleased to support their mission.”
