St. Joseph's Westgate Health Information Management Department maintains all patient medical records.
The medical record is a permanent document of the history and progress of every patient's medical care. The record is a compilation of observations and findings recorded by the patient's physician and members of the hospital healthcare staff, the entries and reports originate at various points in the Hospital. Physicians' dictated medical records are transcribed and added to the medical record.
Records are used for continuity of patient care, verify insurance claims for medical care rendered, as a legal/business document outlining the course of a patient's medical care, provides statistical and factual information for hospital administration and medical staff committees, for hospital accreditation, licensing and other regulatory bodies and medical research.
Patients must request their records in writing.
If you would like to request a copy of your medical records for yourself, please use this request form Westgate or Westgate.
If you need a copy to be sent to a physician's office or other third party, please use this form in St. Josephs or St. Josephs.
Print a copy of the form, fill it out and return to the Medical Records Department at St. Joseph's Westgate.
The Medical Records Department will verify your identity by photo I.D., signature, or pertinent questions. We do this to protect the privacy and confidentiality of your records.
Our department has 30 days to respond to your request and make your records available to you.
