My Care (Patient Portal)


My Care (Patient Portal) provides a convenient, secure, and electronic way to access health information and communicate with your Dignity Health care team. All you need is Internet access and an email account.

How to Enroll

  • Request a My Care (Patient Portal) account during your office visit.
  • Your office may have you complete and sign a request form.
  • Receive an email invitation with a link to complete your enrollment.
  • Once logged into My Care (Patient Portal), bookmark the website.
