We're glad you selected a Dignity Health Medical Group physician as your health care provider. Our physicians are dedicated
to providing you with high quality, compassionate medical care tailored to your personal needs. We are here to serve
Urgent Care is defined as same-day treatment for a non-emergency medical condition. To seek same-day care, call your Primary
Care Physician's office to schedule an appointment. If your Primary Care Physician is unable to see you that day, you
may be referred to a nearby urgent care.
In order to have records from other providers released to your Dignity Health Medical Group provider, please print, complete,
and submit this records release form to the sending provider.
Please arrive 15-30 minutes early to allow time for parking and filling out any paperwork. If applicable, please remember
to bring your parking ticket for validation.
If your physician is located at Dignity Health St. Joseph’s,you may park in the 6th Avenue Garage or the 3rd Avenue Garage,
depending on the location of your provider’s clinic office. There is also valet parking available for a fee.
Many insurance plans require a mandatory prior authorization before a specialist can be seen by a patient. Please check with
your insurance company to see if this is required on your plan before you come for your appointment. Your provider’s
office will have a staff member who handles prior authorizations and will be able to answer any of your questions.
Prior authorization must be received prior to your scheduled appointment. Failure to receive prior authorization/referral
may result in the need to reschedule your appointment to a later date. Please contact your insurance company for more

The practice telephones are answered during business hours. Our telephone staff is prepared to handle your general questions
and will direct as appropriate. After hours and on weekends or holidays, our answering service is here to assist you.
If necessary, they will contact the on-call provider.
Please do not fax any time-sensitive communication or urgent medical advice questions to the office. This could cause
a delay in the response to your time-sensitive communication.
Dignity Health Medical Group has implemented an Online Patient Center.
Please call the office directly to discuss any concerns about your medical condition or if you have questions about your
medications or laboratory results. If your call is of an urgent nature, please call the main number and inform the staff
that you need urgent assistance. REMEMBER, for a true medical emergency do not wait for a return call from the office.
Instead, call 9-1-1 to obtain immediate access to your local emergency medical service.
You will receive an automated telephone call to remind you of your appointments. Please listen to the message and select
one of the following message options:
If you do not wish to receive an appointment reminder, please contact the front office staff.
Should you need to cancel or reschedule an appointment, we kindly ask that you notify us as soon as possible and no later
than 24 hours prior to the appointment so we can reschedule you in a timely manner and offer the open slot to another
patient. If you are late for your appointment, you may have to be rescheduled. Multiple cancellations or failure to show
will impact our ability to care for you.
For refills on medications prescribed by a doctor from this office only, please ask your pharmacy to fax a refill request
to our office directly. It is important that you do not run out of your prescribed medications. Please plan ahead for
refills and allow three business days for faxed refill requests to be processed and returned to the pharmacy. Keep in
mind that prescriptions for certain controlled substances require a hard copy prescription that cannot be called in or
faxed. It is the policy of our doctors not to refill controlled substance prescriptions for our practice partners; therefore,
you must obtain these prescriptions from your specific physician.
Your doctor may order diagnostic tests as part of your evaluation and care. Most insurance companies require prior authorization
and approval before your test can be scheduled. Your doctor will write orders for these tests and the staff in our referrals
and authorization department will obtain proper authorization from your insurance company before the test can be scheduled.
Our staff will also assist in scheduling this test for you. If you have questions about upcoming test scheduling, you
will be directed to the appropriate referrals and authorization staff member handling your case.
