Records are used for continuity of patient care, to verify insurance claims for medical care rendered, as a legal/business document outlining the course of your medical care, to provide statistical and factual information for hospital administration and medical staff committees, and for hospital accreditation, licensing and other regulatory bodies and medical research.
Your medical record is a permanent document of the history and progress of your medical care. The record is a compilation of observations and findings recorded by your doctor and by members of the hospital healthcare staff. Your doctor's dictated medical records are transcribed and added to your medical record as well.
To have your medical records sent to a provider, please complete an Authorization for Use or Disclosure of Protected Health Information form.
