Notice of Records Being Compromised

Dignity Health Medical Foundation Protected Health Information Incident

Mercy Medical Group, a Service of Dignity Health Medical Foundation, became aware of an incident that resulted in compromise of some protected health information.  Dignity Health Medical Foundation is notifying patients whose records may have been compromised. 

On September 6th, we learned that an employee who moved out of her home inadvertently left behind papers containing administrative information and some protected health information. The information contained in the papers included at least some of the following: demographic information and diagnosis information.   The papers were found by a third party and promptly returned to Mercy Medical Group.

Because the papers were returned promptly, we have no reason at this time to believe that the information has been used inappropriately.  

Mercy Medical Group regrets any inconvenience caused by this incident.  Letters have been mailed to patients whose medical information was included.  Mercy Medical Group has established a call center to answer any questions affected patients may have. 

Individuals who were patients at Mercy Medical Group Cardiology Clinic from January 1, 2012 through December 31, 2012, can call (833) 684-0205 to obtain additional information.
