California Hospital goes beyond the acute-care setting of our hospital to help improve the long-term health of the communities we serve.
We believe in supporting healthy living in ways that extend far beyond the walls of our hospital. That's the reason why we have invested $35 million per year in charity care and community benefits. Truly good health results from education, nutrition, and people working together to invest in their communities. There are so many ways that we can support our patients once they return home, through mental health resources, home visits, educational classes, nutrition seminars, and after school programs. Funded primarily through philanthropic support, our comprehensive array of outreach services empower children and families to take charge of their health and the health of their communities.
At Dignity Health, a key part of our mission is supporting the health of our communities in meaningful, measurable ways. It is also part of our mandate as a non-profit healthcare provider under the Patient Protection and Affordable Care Act. That is why each and every one of our hospitals goes to great lengths to study and ascertain the most pressing health needs of the communities they serve. This process and its conclusions are described in a document called the Community Health Needs Assessment, which is updated every three years.
California Hospital Medical Center and JWCH Institute securely share patient information to support the FUSE Program as allowed under the law as an organized health care arrangement. Sharing information as an organized health care arrangement furthers the FUSE Program’s goals of improving the quality of care and reducing costs.
