Physician Integration

Dignity Health works closely with affiliated physicians through a variety of collaborative physician integration models. These include medical practice foundation and exclusively contracted medical group models, affiliated medical group models, clinically integrated networks, and Independent Practice Associations across California, Arizona, and Nevada.

Medical Groups and Health Centers
In California, there are 14 medical groups that are affiliated with Dignity Health Medical Foundation (DHMF), representing 120+ clinic locations across the state of California that deliver high quality, affordable, and patient-centric primary and specialty care. On the California central coast, Pacific Central Coast Health Centers (PHC), a service of Dignity Health, is a non-profit clinic organization comprised of more than 30 community health centers located in cities throughout Northern Santa Barbara County and San Luis Obispo County. As a Dignity Health member, PHC is a part of Dignity Health Central Coast’s award-winning network of hospitals, imaging centers, laboratories, post-acute centers, and expert physicians. In Arizona, Dignity Health Medical Group is part of Dignity Health’s Arizona regional hospital system. This unique relationship with the hospital system allows Dignity Health Medical Group to provide its patients with state-of-the-art patient services. Independent Physician Associations
Dignity Health Medical Network (DHMN) is Dignity Health Medical Foundation’s managed care health plan contracting entity for independent physician associations (IPAs) in California, providing the support and resources to help IPAs better serve their managed care patients. It partners with providers, hospitals, and health care delivery systems to create networks that deliver value for the communities they serve. DHMN currently supports independent practices in Santa Cruz, the Bay Area (Sequoia), and Bakersfield representing more than 600 independent providers.

Clinically Integrated Networks
Dignity Health has built clinically integrated networks across three states, covering a variety of diverse demographics and geographies. Our clinically integrated networks represent thousands of providers, more than three dozen hospitals and many joint venture partners that are aligned under a patient-centric model that is focused on providing high quality, affordable care with a focus on doing what is best for each of our patients and all of the communities we serve.
