An AWV helps you and your primary care provider create a personalized prevention plan tailored to your unique needs. It gives you a clear picture of your current health, identifies any recommended tests or screenings, and helps you take proactive steps to maintain your well-being. Unlike a standard physical or check-up, an AWV is a yearly appointment focused on prevention, designed to support better health today and help you head off potential illnesses in the future.

 

What to expect at your Annual Wellness Visit

  • How to prepare

    Bring a list of your questions and what medications you’re taking so you and your primary care provider can create your personalized prevention plan.

  • Your time to ask

    Unlike a physical, this visit is about you connecting with your doctor. It’s your chance to ask questions, understand your risk factors, and prioritize your ongoing health.

  • Personalized prevention plan

    During your visit, your'll be asked to fill out a Health Risk Assessment (HRA), a questionnaire that can help your provider develop a personalized prevention plan. 

  • What's included

    It covers a review of your personal and family medical history, along with a focused physical exam featuring vital signs, screenings, and cognitive assessments to provide a complete picture of your health.

  • Other vaccines & immunizations

    Learn more about adult immunizations and make sure you’re up to date. Setting time aside for a visit with your health care professional allows you to get your vaccine questions answered sooner rather than later.

  • What happens next

    Before you leave, jot down any next steps your provider recommends—whether it’s scheduling a follow-up appointment, or checking in with a specialist. If you have questions after the visit or want to share an update, give them a call.
