The term “medical home” can mean many different things. At Dignity Health Medical Group, it has been defined as a process by which the provider and patient are cooperatively involved in ongoing healthcare management, working towards the goal of high quality, cost-effective medical care. It is integrated care centered around each individual patient.
The goals of our PCMH-recognized outpatient practices are to provide patients with:

- Care that looks at the whole person, including behavioral health and social issues
- Coordinated and integrated care from primary medical care to subspecialty needs
- An approach focused on high quality care and a safe environment
- Enhanced access to care outside of traditional business hours
The medical home amplifies patient care that is designed to help them control their current health issues and prevent future issues while deterring them from using the emergency room as their primary healthcare resource.
