Referral contact information

If you are a medical provider or office staff who is referring a patient to our office, please and ask to speak with the Physician Referral Coordinator.   Most patients can be seen within 1-2 weeks of their initial referral. 

Our commitment to referring physicians

Our commitment is to provide a timely consultation to referred patients, while supporting the clinician throughout the referral process. 

We provide timely communication regarding your patient's care through the initial consultation, and any subsequent surgical procedures.  You will receive a copy of the surgeon’s consultation within 24-48 hours after the patient is seen in our office. 

If the patient undergoes surgery with our team, we will provide ongoing communication throughout the perioperative period. Patients will be followed postoperatively in our clinic within 7-10 days after hospital discharge.  

Ongoing communications will include;

  • A call from our surgeon updating the referring clinician at the completion of the surgical case.

  • Copies of clinical notes will be faxed to the referring clinician within 24-48 hours of the patient’s appointment. 

  • Once final follow-up visits are complete, a comprehensive clinical note will provide information including any further recommendations for continued care.

The referral process

The Physician Referral Coordinator (PRC) acts as your resource for referring patients to the Dignity Health Cardiovascular and Thoracic Surgery specialty practice. The PRC at our office will take basic patient information and will immediately start the process to coordinate the patient’s office consultation.  

Once the referral has been made, the PRC will provide the patient with the necessary information for their upcoming consultation, including scheduling, appointment confirmation, parking information and a map to our offices.  

The PRC will also help to:

  • Schedule the patient consultation quickly to ensure minimal waiting for consultations. 

  • Work with the patient and referring office to help gather any needed medical records and pertinent imaging prior to the patient’s consultation.

  • Identify the patient’s insurance carrier and coordinate insurance issues related to their visit. 

  • Provide information about clinic and hospital surgical services. 

  • Facilitate clinician to clinician communication throughout the referral process.

  • Coordinate postoperative scheduling for ongoing patient follow-up. The initial postoperative follow-up visit is scheduled for 7-10 days after hospital discharge.

Self referrals

We welcome patients who decide to see one of our providers in consultation. Patients who are referring themselves to our office follow a similar process as described above. Please contact our office and speak with our patient referral specialist regarding scheduling a consultation with one of our surgeons.
