What is the Online Patient Center?
My Care (Patient Portal) is a website that allows you a convenient and secure way to communicate with your doctor's office and view portions of your medical records online. Within your My Care (Patient Portal) account, you will be able to do the following:
How can I create an account?
At your next appointment, tell the staff you would like to have a My Care (Patient Portal) account. The staff will request your email address, and have you fill out a consent form. In most cases, you will receive an email with a link and your personal PIN within 2 business days. Just click on the link and follow the instructions to complete your enrollment. Some offices are providing onsite account creation. Ask your doctor's office if it is available there.
Can I create an account for a family member who has a Dignity Health doctor?
Children and dependent adults who have a Dignity Health doctor may have a My Care (Patient Portal) account with you as the proxy. However, for dependent children, only those children under the age of 12 may have their records viewable with a My Care (Patient Portal) account. For a dependent adult, you will need to provide a copy of the Medical Durable Power of Attorney before an account can be created for the dependent adult.
Who has access to my account?
Some of the information in your My Care (Patient Portal) account is provided through your medical health record and is viewable by you and your doctor's office. In addition, if you give your log-in and password information to others, they will be able to have access to your account. My Care (Patient Portal) allows you to add additional information related to your health to your My Care (Patient Portal) account. If you do so, you'll have your information in a secure place that can be accessed anywhere you have Internet access. Please note that the information you add to your My Care (Patient Portal) account is viewable only to you. It does not update your medical health record at your doctor's office and your doctor cannot view the information you add.
If my PIN expires or is lost, how can I request a new one?
