What is a Medical Record?

The medical record is a permanent document of the history and progress of every patient's medical care. The record is a compilation of observations and findings recorded by the patient's physician and members of the hospital health care staff. The entries and reports originate at various points in the hospital. Records are used for continuity of patient care, to verify insurance claims for medical care rendered, and as a legal/business document outlining the course of a patient's medical care.

We are prohibited from giving any protected health information (PHI) over the phone under any circumstances, including COVID test results. 

Access Your Patient Records Online

Dignity Health now offers an Online Patient Center for our hospital patients. You or a designated family member can view your health records, download them or send them to your personal physician. Proxy access can be obtained at Health Information Management. For help enrolling, just ask us.

Visit DignityHealth.org/Patients and select St. John's Regional Medical Center from the drop-down menu to log on and access your hospital rel="noopener noreferrer" patient records.
