Yavapai Regional Medical Center - Patient Medical Records

Our Medical Records Department provides support to all patient care areas within the hospital, our urgent care facilities and laboratory.

The staff is committed to protecting and maintaining health records to support the delivery of patient care at Dignity Health Yavapai Regional Medical Center, Yavapai Regional Medical Group, and Dignity Health Outpatient Imaging Center in Prescott and Prescott Valley. 

Our team also provides information for birth certificates and death certificates, assists with amendments to health records, and helps patients set up their YRMC CareConnect accounts to access health records online.

YRMC CareConnect (Patient Portal)

When you receive care at Dignity Health Yavapai Regional Medical Center in Prescott or Prescott Valley or visit your primary or specialty care provider at any Yavapai Regional Medical Group (YRMG) clinic, important information is being added to your medical record.

Your health care team has access to this important information, and with YRMC CareConnect, you have access to much of the same information anytime, anywhere you have an Internet connection.

With YRMC CareConnect, you can:

  • Access your medical record, including procedures, lab results, imaging reports, immunizations, medications, and allergies
  • Review care summary and discharge documents
  • Download elements of your medical record to easily share with other health care providers
  • Send a message to, or request an appointment with a primary or specialty care provider at a YRMG clinic
  • Check on upcoming appointments at Yavapai Regional or a YRMG clinic
  • Receive reminders about important health care items

You can also enroll when you visit a provider at any YRMG clinic or when you register for care at Dignity Health Yavapai Regional Medical Center in Prescott or Prescott Valley.

  • You can access you medical record information using the HealtheLife Patient Portal app, which is available for download to your mobile device from the Apple App Store and from the Google Play Store.
  • You can also use the Apple Health app on your Apple device. Please follow these instructions to configure your Apple Health app for use.

    We recommend that you check back periodically for information about additional apps that may become available.

Resetting your password is simple. Just visit the log-in page for YRMC CareConnect and click on “Forgot Your Password.” Then follow the instructions to reset your password.
