Preparing for Hospitalization

Registration

The Admitting Department at St. John's is here to assist you whether you come for an inpatient stay, outpatient services, or for emergency room services. At registration, you will be asked to confirm all of your demographic information such as name, address, Social Security Number, and date of birth. We do this to protect your privacy and to make sure we have the correct information to bill your insurance properly.

To expedite your service, please have the following documents ready:

  • Insurance card
  • Doctor's orders
  • Photo identification
  • Authorization letter (if required by your insurance provider)

Advance Care Directives

During the registration process, you will be asked if you have an advance directive. As a patient, you have the right to be fully informed about treatments, alternatives, benefits, and risks relating to health care. You also have the right to consent or withhold consent to any health care option, including life-sustaining treatment. Your right to express your wishes in the event that you become incapacitated or unable to communicate can be preserved by executing documents in advance, called advance directives for health care. Advance directives can include a health care power of attorney, health care directive, or an anatomical gift organ donor designation. If you do not have an advance directive and wish to obtain one, St. John's admissions registrars will be happy to provide you with one, or to provide you with more information. Advance Beneficiary Notices

You may receive a Medicare advance beneficiary notice (ABN) from your physician. When your doctor wants to diagnose your condition or evaluate how well your treatment is working, he or she may order laboratory tests. Medicare may refuse to pay for these tests. If this happens, your physician will ask you to sign an ABN. You will need to bring this to the hospital and present it upon registration.
