    On April 24, 2018, Dignity Health discovered that an emailing list formatted by Healthgrades, a business associate, contained a sorting error. This sorting error caused an email notice intended for a select group of patients to be sent to the wrong person. The email was meant to notify recipients that they can now schedule appointments online with their physicians.
