Dominican Hospital offers a nationally certified Cardiac Rehabilitation program designed to support recovery after a heart attack, heart surgery, or other cardiac event. This medically supervised program combines exercise, education, and emotional support to help you regain strength, reduce your risk of future heart problems, and improve your quality of life.

Diabetes Management
Dominican Hospital’s Diabetes Management Program is certified by the American Diabetes Association (ADA), meeting national standards for high-quality, evidence-based diabetes education and support.
If you’re living with diabetes, we’re here to help. Our team of Certified Diabetes Care and Education Specialists, registered dietitians, and nurses provide individualized guidance to give you the tools and support you need to take control of your diabetes, prevent complications, and improve your quality of life. Based on your doctor’s referral, you may be scheduled for one-on-one visits, a comprehensive diabetes class series, or both.
Our program includes:
- Diabetes Self-Management Education and Support (DSMES)
- Personalized education plans tailored to your medical needs and goals
- Support with blood sugar monitoring, medication, and insulin use
- Guidance on healthy eating, physical activity, and stress management
- Tools to help you build confidence and make lasting lifestyle changes
Medical Nutrition Therapy (MNT)
Our Medical Nutrition Therapy program offers individualized nutrition counseling from registered dietitians to support long-term wellness and effective management of medical conditions.
Each session is tailored to your specific health goals, lifestyle, and medical needs to improve nutrition and reduce health risks.
MNT can support patients with the following:
- Weight loss or bariatric surgery preparation
- High cholesterol and high blood pressure
- Chronic kidney disease and digestive disorders
- Eating disorders and other medical nutrition concerns
- Prediabetes
Supervised Exercise Therapy for Peripheral Artery Disease (SETPAD)
Our SETPAD program is a structured, evidence-based exercise therapy for individuals with Peripheral Artery Disease (PAD) and intermittent claudication.
This Medicare-covered program improves walking ability, reduces pain, and supports vascular and cardiovascular health.
SETPAD includes:
- Supervised treadmill walking and endurance training
- Personalized exercise plans with clinical oversight
- Education on lifestyle changes to reduce cardiovascular risks
- Support to delay or prevent the need for surgery
