Helping You Manage Your Diabetes

We offer a variety of programs, both group and individual appointments, to help you stay healthy.

Blood Glucose Meter Instruction

Everyone is welcome, but an appointment is required. A blood glucose meter will be provided for free, and an educator will instruct on blood glucose monitoring and goals. (No fee.)

Comprehensive Self-Management Course

This course is designed for those with type 2 diabetes. Techniques and strategies are presented to optimize diabetes self-management. (Fees apply.) 

Grocery Store Tour

Enjoy a comprehensive grocery store tour and learn how to incorporate new or favorite foods into your meal plan. Held at Trader Joe’s in San Carlos. (Fees apply.)

Individual Appointments

We offer individual appointments with a nutrition educator or nurse educator to address specific needs. (Fees apply.)

Insulin Pump Initiation

This program is designed for external insulin pump initiation. Individuals will begin using the insulin pump and will be trained on aspects of pump therapy. (Fees apply.) Information will include:

  • Pump therapy principles and operations
  • Dose calculations
  • Pattern management
  • Advanced carbohydrate counting skills

 

Intensive Insulin Treatment

This program is for those who are taking insulin and is based on intensive insulin management principles. (Fees apply.) Topics include:

  • Multiple injection therapy guidelines
  • Insulin dose adjustment
  • Blood glucose pattern interpretation
  • Problem solving skills
  • Carbohydrate counting skills

 

Pre-Diabetes Courses (Individual and Group Courses)

This course is for those with pre-diabetes or metabolic syndrome. It has been proven that with small lifestyle changes we can decrease the risk of developing type 2 diabetes. Content will include lifestyle modification strategies that lead to a healthier way of living. (Fees apply.)

Support Groups

We offer support group sessions for adults with diabetes. These sessions provide a place for people with diabetes and their loved ones to gather, share, and ask questions about how to manage a condition that can be demanding and challenging. Guest speakers may be featured at some meetings. For meeting information, please call  (650) 367-5213. (No fee.)

Sweet Success Pregnancy Program

If you have gestational diabetes or diabetes and are pregnant, our Sweet Success program helps you manage your diabetes throughout pregnancy. We look forward to giving you the information and support you need to have the best possible outcomes for you and your baby. (Fees apply.) Our program includes:

  • Blood glucose monitoring
  • Exercise guidance
  • Individual follow up sessions throughout pregnancy
  • Individualized meal plan
  • Nutrition goals
  • Preconception planning for type 1 and type 2 diabetes

Weight Loss Program

Participants in this series will initially meet individually with a registered dietitian, certified in diabetes education, for ½ hour. Weekly one-hour meetings will include ongoing support via weigh-ins, food log
reviews, telephone support, group support and helpful weight loss tips and strategies. (Fees apply.)
