Community Wellness Program

Asthma Management Program

This asthma home visiting program is provided at NO COST to eligible individuals with poorly controlled asthma. The program provides education, home asthma trigger assessment, and home asthma trigger remediation. Eligibility requirements include:

  • Kern County Resident
  • 6 years of age or older
  • Diagnosed with poorly controlled asthma
  • Medi- Cal insurance

Active Living Program

Walking for Fitness: In this six-week series, participants will utilize the American Heart Association program, Walking 101, to start a physical activity plan of their own. Each session consists of education and group walks.

Healthier Living Program

The Community Wellness Program offers evidencebased self- management programs which are designed to keep you healthy and mentally engaged. These programs teach practical everyday skills such as

  • Techniques to deal with diabetes, fatigue, pain, stress, and frustration.
  • Ways to boost stamina with fun endurance workout.
  • Healthy eating and menu planning.
  • Communicating effectively with family, friends, and health professionals.

Our evidence – based programs include:

  • Enhanced Fitness

Health Education Program

This program provides education on the following chronic conditions.

  • Asthma: This class will help improve your understanding of asthma, its triggers, and how best to prevent attacks. 
  • Cancer: Learn about risk factors and preventative steps to lower your risk. Education is provided on: Breast Health, Colorectal Cancer, and Cervical Cancer. Cancer screenings and/ or resources are provided in conjunction with education.
  • Diabetes: This class will help improve your understanding of diabetes, risk factors, and the role of nutrition and exercise in prevention.
  • Heart Health: Learn about the risk factors of heart disease and what preventative steps you can take to lower your risk for these chronic conditions.
  • Nutrition Education: Our nutrition classes and workshops provide education on the “My Plate” approach to good nutrition, meal planning and preparation, reading food labels, and shopping on a budget.

Health Screening Program

Health Screenings: Free monthly health screenings are offered throughout Kern County! Get your glucose, blood pressure, cholesterol, and hemoglobin measured at a location near you. Results are provided within minutes. Locations include:

Healthy Kids in Healthy Homes

This  nutrition and fitness education program for elementary school children provides a science- based approach to nutrition and fitness. In this program, children learn to identify healthy foods and to increase their level of physical activity—two tools proven to prevent childhood obesity.

Youth Tobacco Prevention Program 

This four-week workshop is designed to prevent students from starting or escalating their use of tobacco and nicotine products. The program consists of 4 sessions designed to help students gain an understanding of tobacco products, including e-cigarettes and vape pens, the harm they cause, the deceptive marketing strategies tobacco manufacturers utilize, and refusal skills.
