Request for Transcripts


For transcript requests, please email [email protected] with the following information:

  • Spell first and last name
  • Date range for transcript request  (for example, January 1, 2008, through today)
  • Specify address, email and fax number to send report to
  • Provide a phone number or email address for clarification purposes

CME transcripts are available 30 days after the close of the event. Please allow five business days to receive your transcript of CME activities. Please call the Office of CME at (480) 728-9990 with any questions.
