Make your admission to the hospital much easier by pre-registering. This gives us time to gather important information about you before you arrive at the hospital. You can pre-register online, by mail, phone or at the hospital.
Print (Please print the form, complete the information and return it by fax or bring it with you):
Please note that when you arrive at the facility, you will still be required to provide your identification, finalize financial arrangements and sign a few forms before going to the department.
For questions or concerns about your portion of the hospital bill, please contact Billing Office at 480.728.4300 or via email [email protected]. If you need information about Payment Assistance Programs such as AHCCCS/Arizona Department of Economic Security government programs, please call 480.728.2966.
When you arrive at Chandler Regional Medical Center, located at 1955 W. Frye Road Chandler, AZ 85224, follow the signs for the Family Birth Center entrance. Once you enter, the nurse at the admitting desk will make arrangements for you to go to triage. In triage, you will have an initial assessment which may include fetal monitoring to determine if you are in labor. Depending on your condition, you will be admitted for delivery or you may receive instructions to come back to the hospital once your labor has progressed.
Knowing what to bring ahead of time can make your trip to the hospital less stressful and your stay more comfortable. Our list of items to pack will help you prepare.
Before you go home from the hospital, you will need to complete the forms to receive your baby’s birth certificate and social security card. This fact sheet will help you gather the information needed to properly complete those forms.
If you have any questions regarding registration, please contact our birth registrars at 480.728.7417, Monday through Friday, 6 a.m. to 2:30 p.m.
