Dignity Health Now Accepting Applications For 2026 Community Health Improvement Grants

PHOENIX (July  23, 2025) – Dignity Health Yavapai Regional Medical Center (YRMC) is pleased to announce that applications for the 2026 Dignity Health YRMC Community Health Improvement Grants are now open. Any non-profit 501(c)3 organization in the Yavapai County area may apply to be one of the lucky recipients.

Funding will be given to projects that address one or more significant health needs in the hospital’s most recent community health needs assessment. Programs should have an emphasis on health equity by way of providing access to care, healthy lifestyle and behaviors, prevention and social determinants of health.

Dignity Health YRMC’s Community Health Improvement Grants program is designed to support collaborative efforts that improve the health and well-being of vulnerable and underserved populations in the communities we serve. Last year, YRMC gave $239,000 in community grants to local organizations.
