Welcoming Our New Patients
To ensure new members are fully set up in our care network, we offer a “Welcome to Dignity Health” virtual onboarding appointment. During this virtual session, we assist members in completing their health profile, address any questions regarding our services, help set up their patient portal and schedule a new patient visit appointment with their primary care physician.
Integrating Wellness Into Our Community
We recognize the strong relationship between our lifestyle habits and our health. The Dignity Health Wellness Center of Dominican Hospital offers a wide range of health, wellness and rehabilitation classes, both in-person and virtual, for people of all ages and abilities. We are staffed with a team of highly skilled clinical specialists – exercise physiologists, registered dieticians, registered nurses and respiratory therapists.
As an additional benefit to our patients, we have taken the step to integrate referrals to the Wellness Center into electronic medical health record. This allows physicians and other primary care providers to have a direct link with Wellness Center staff, allowing for immediate communication regarding programs, individual progress and outcomes.
