MSSP
Our Multipurpose Senior Services Program (MSSP) helps frail, Medi-Cal eligible seniors (60+) in 8 NorCal Counties maintain independence at home. Through personalized case management, we connect you with essential services like meals, transportation, and nursing care to support your well-being and prevent nursing home placement.

Enhanced Care Management
Enhanced Care Management (ECM) offers Partnership Healthplan members free, coordinated, whole-person care. Our Community Healthcare Workers work with you to address health and daily living needs, providing personalized support to navigate the health system and achieve your individualized goals.
