UniHealth Foundation Grant

Recently, the UniHealth Foundation awarded $601,296 to Glendale Memorial Hospital. This grant is to be used for stroke care over the next three year period.

The purpose of the grant is to support the launch and sustainability of a transitional care program designed to prevent secondary stroke and facilitate quality of life. The program will be nurse-led and provide enhanced care coordination to stroke patients following discharge. Additionally, the program will offer culturally appropriate stoke prevention education to the community.
