Make your admission to the hospital much easier by pre-registering. This gives us time to gather important information about you before you arrive at the hospital.
You can pre-register online now, or by calling 602.406.3521. Registration forms can also be downloaded below or picked up at the hospital.

Please note that when you arrive at the facility, you will still be required to provide your identification, finalize financial arrangements and sign a few forms before going to the department.
When you arrive at St. Joseph’s Hospital and Medical Center, located at 350 W. Thomas Road, Phoenix, AZ 85012, follow the signs for the Labor and Delivery entrance. Once you arrive, the admitting staff will make arrangements for you to go to triage in the Family Birth Center. In triage you will have an initial assessment which may include fetal monitoring to determine if you are in labor. Depending on your condition you will be admitted for delivery or you may receive instructions to come back to the hospital once your labor has progressed.
Knowing what to bring ahead of time can make your trip to the hospital less stressful and more comfortable. Our list of items to pack will help you prepare.
Before you go home from the hospital you will need to complete the forms to receive your baby's birth certificate and social security card.
