Get your Wellness Blood Panel drawn annually. Follow the link below to get your lab slip from employee health. Take your lab slip to our hospital lab between 7:00 – 11:00 AM to get your blood drawn at no cost to you. Remember to take your employee ID. Results will be provided to you by employee health within 3-4 days. If you have already completed an annual lab panel that includes a lipid panel and HbA1c, you can input those results into the online Health Assessment. Complete an online Health Assessment and get a personal and private report evaluating your risks and potential impact of current lifestyle choices on your future health. Follow the link below to enter your confidential information regarding you lifestyle choices, current conditions, lab results, The Health Assessment will ask you to enter your lab results. (If you don’t have them yet, you can enter them later.) When you have completed your assessment, you are given the option to print your multipage report and make future lifestyle choices to maintain or improve your health.
