Home Health

Mercy Medical Center's Home Health program will help you and your family manage your health care needs when you are well enough to be discharged from the hospital, a nursing home, or other inpatient care setting. It can also be used provide you with the medical treatment so you do not have to be admitted into inpatient care.

Am I Eligible for Home Care?

Admission to Mercy Home Care’s program must come at your doctor’s request and is based on your current medical condition. Your doctor will determine which types of care you need to continue the recovery process, regain self-sufficiency, or remain safe in your home.

Professional Care Teams

Our home care professionals are all state-licensed, Medicare-certified and JCAHO-accredited. Our clinically skilled home care teams may include:

  • Certified Home Health Aides
  • Medical Social Workers 
  • Occupational Therapists
  • Physical Therapists
  • Registered Dietitians
  • Registered Nurses
  • Speech Therapists

Home Health Services

  • Assistance with daily living 
  • Medical social work
  • Physical therapy
  • Skilled nursing
  • Specialized services for cancer care, infusion therapy, wound care, and enterostomal therapy


We are a preferred provider for Blue Cross and Blue Shield and accept the following:

  • CCAH
  • Medicare 
  • MediCal
  • Most private insurances

Before starting our care, we will notify you of the home care coverage that you have.
