Patient Navigators Help Medi-Cal Patients Find Continuing Care


More than 50 percent of all patients seen and treated in Dignity Health's hospital emergency departments are showing up for basic primary care needs.

Dignity Health, in partnership with Sacramento Covered and Health Net, provide a patient navigator program in our emergency departments targeting Medi-Cal insured and uninsured, to ensure patients timely access to appropriate and quality care. The goals of the Patient Navigator program are to ensure patients are able to access timely, appropriate primary care in the community, decrease dependence on emergency departments for basic care and reduce cost. KCRA 3 visited Dignity Health Methodist Hospital to see how the program is working.
