The following visitation guidelines apply to all of our patients:
In special circumstances, we reserve the right to impose additional restrictions. These situations will be determined on a case-by-case basis.
For all visitors:
If visitors are fully vaccinated (this includes having their booster dose if they are booster-eligible*), they must show proof of vaccination to enter. For a digital COVID-19 vaccination record, visit https://myvaccinerecord.cdph.ca.gov/.
If visitors are unvaccinated or booster-eligible*, but do not yet have a booster, they must show proof of a negative antigen test taken in the last 24 hours or a negative PCR test taken within the last 48 hours.
For visitors who visit for multiple consecutive days, proof of a negative test is only required every third day (therefore testing is only required on day one, day four, day seven and so on).
In emergent situations, there is a 72 hour grace period during which the visitor is exempt from the vaccination and testing requirements, however the visitor must comply with the requirements as soon as reasonably possible.
*Booster eligibility is defined as 5 months past the Pfizer or Moderna two-dose series, or 2 months past the single dose of Johnson & Johnson.
