Home Health & Hospice 


Dignity Health in Greater Sacramento has a long tradition of caring. Our commitment to compassion is reflected in our home health and hospice care programs. Our Home Health and Hospice programs bring together experienced care teams of nurses, therapists, aides and medical social workers who understand the importance of home health in the care continuum and who have the clinical skills to meet all home health and hospice care needs of our patients.

Home Health Care

Dignity Health’s Home Health programs offers a full range of quality home health care options for patients recently discharged from the hospital, as well as homebound individuals throughout the Greater Sacramento region who need additional home health care.

Our home health program also provides care for mothers and babies, special needs children and other patients referred by doctors, insurers, hospitals and community agencies.

The Mercy Hospice program provides special care for terminally ill patients and their family members. Mercy Hospice provides physical, emotional and spiritual comfort and care in the supportive surroundings of the patient's place of residence.
