Community Programs


Serving our vibrant community is at the heart of our mission. We're proud to go beyond hospital walls to help improve the health of our community by offering programs to care for the mind, body, and spirit.


Community Health Department

St. Joseph's is working to build healthier communities through education, outreach services and support. We also welcome those who want to join our mission by applying for grant, spreading the word about our community classes and events, and volunteering to serve their community. 

Providing Education, Medical Services & Support

Check out our Classes & Events to view a listing of all of the various classes and events we offer to our community. We offer educational diabetes workshops in English, Spanish and Lao. We also provide basic medical care to those without access to care on our CareVan Mobile clinic. Peer-led support groups like Stanford's Healthier Living Program offer help for those living with chronic diseases. 

Community Grants Program

Through our Community Grants Program we partner with other nonprofit organizations whose programs respond to priority health needs identified through our Community Health Needs Assessment. Grant funds are used to provide services to underserved populations, specifically Communities of Concern identified in our county. Organizations that receive grant funding are working to improve access to health care, jobs, housing, food, and education for people in low-income and minority communities.

Since the program began in 1990, St. Joseph's has contributed more than $2.2 million in grant funding to deserving organizations, and Dignity Health as a whole has contributed $51.3 million. ER Navigator Program & Familiar Faces Initiative

St. Joseph’s Emergency Room routinely sees about 70 non-urgent patients daily, often due to a patient’s lack of connection with a primary care physician for basic medical services. In an effort to link patients with the right level of care and/or proper services for their needs, St. Joseph’s has adopted the ER Navigator Program and the Familiar Faces Initiative. 

The ER Navigator helps to connect patients with community resources, eliminates barriers such as language and lack of transportation, and helps patients understand the role of primary care providers vs. the emergency department.

St. Joseph’s Familiar Faces Initiative, which is tailored to patients who have 24 or more visits per year, seeks to help patients regain stability and overall health. Coordinators with Community Medical Centers meet with patients following their medical treatment at St. Joseph’s, arranging for transportation between the ER and their community service clinic on Waterloo Road. This community service clinic then become the patient’s home base for medical, social, and behavioral health services.
