Start the registration process and avoid the extra time needed to complete paperwork upon arrival:
Pre-register your stay at your convenience using our simple, online form. All information submitted in the online pre-registration form is treated as confidential.
You can also print the pre-registration form, complete the information and return it by fax (602.406.4139) or bring it with you at check-in:
Upon receipt of your form, a staff member will contact you to go over the information provided to finalize your registration.
Please note, when you arrive, you will still be required to provide identification, finalize financial arrangements, and sign a few forms before going to the birth center.
