How to Request a Copy of Your Medical Record
Patients must request their records in writing.
Dignity Health now offers an Online Patient Center for our hospital patients. You or a designated family member can view your health records, download them or send them to your personal doctor. Proxy access can be obtained at Health Information Management (HIM). For help enrolling, call 209.754.2591.
Visit DignityHealth.org/Patients and select Mark Twain Medical Center from the Central California drop down menu to log on and access your hospital patient records.
