St. John’s Hospitals Award $140,000 in Grant Funding to Community Non-Profit Organizations


Grants to help provide critical health and human services to Ventura County residents
OXNARD, CA – (January 16, 2019) – Dignity Health St. John’s Pleasant Valley Hospital (SJPVH) and St. John’s Regional Medical Center (SJRMC), are proud to announce they have awarded $140,000 in grant funding to three important community non-profit partnerships in Ventura County. Representatives from each organization were invited to a breakfast on January 8, 2019 where they were presented with checks to aid in the critical health and human services they provide to county residents.

Grants are awarded through Dignity Health’s Community Grants Program which sets forth to help non-profit organizations whose efforts aid in building healthier communities by improving health and living conditions.

“Partnering with local organizations who share our vision is essential to improving access to care and the overall well-being of our community,” said Darren Lee, President and CEO of St. John’s Pleasant Valley Hospital and St. John’s Regional Medical Center. “Collectively, our efforts will help the most underserved and vulnerable populations in our community.”

This year’s, Community Grants were awarded to:


Brain Injury Center of Ventura County - $65,000

Program: Extended Care Transition for the Cognitively Impaired

Program Description: This project is the critical “warm handoff” from acute care for patients with complex needs resulting from brain impairment.

Planned Impact: The project addresses the identified need for a seamless continuum of care, from acute-inpatient treatment to community living as well as needs identified by primary care physicians and other non-acute care settings. Whole-person, self-empowering intervention will result in timely, effective use of available community-based supports and care to an underserved population: persons with dementia or cognitive impairment secondary to disease processes or trauma and their caregivers.


Interface Children & Family Services - $50,000

Program: 211 Ventura County Homeless Services Outreach & Coordination

Program Description: Interface will employ two methods of reaching vulnerable community members to broaden and deepen the way we help ensure 24/7 bilingual access to Countywide safety net services for people who are homeless or at risk of becoming homeless.

Planned Impact: Program aims to provide 1,200 community members with benefits screenings and/or linkages to the Homeless Management Information System, with a portion of this population receiving intensive case management. Ultimately, these efforts will increase access to health care, housing, and other social services for previously unidentified youth and adults who are homeless or nearly homeless.


Community Action of Ventura County - $25,000

Program: Homeless Dental Services Project

Program Description: The program provides homeless individuals with basic dental care information and supplies as well as access to safe and sanitary locations for daily dental hygiene. Transportation is also provided to a local dental clinic for no-cost basic and, if needed, non-basic dental services, as well as assistance with identifying a dental home, enrolling (if eligible) in Denti-Cal, and developing a long-term dental care plan.

Planned Impact: Increase access to dental health care services for homeless adults throughout the City of Oxnard and surrounding communities.
