Greater Sacramento Project Proposals
Program Overview
Dignity Health is pleased to announce the opening of the 2026 Community Health Improvement Grants cycle for the Greater Sacramento Market. This grant program is one way we are working with community partners to improve the health and well-being of vulnerable populations in our community. As such, Dignity Health intends to award up to $1.1 million in Community Health Improvement Grants to support 501(c)3 organizations that provide programs and services that seek to address access to care and resources, chronic disease prevention and treatment, and mental health and substance use in Sacramento and Yolo Counties. Each selected organization will be awarded between $20,000 - $100,000 for projects lasting 12 months, beginning in April 2026.
Application Process
Eligibility & Selection Criteria:
- Proposals must address one or more of the significant health needs identified in the Sacramento and Yolo County 2025 Community Health Needs Assessment (CHNA):
- Access to Care
- Mental Health & Substance Use
- Chronic Disease Prevention & Management
- Access to Resources
- Must have two or more collaborating partner organizations.
- Proposals should demonstrate how projects address unmet health needs, emphasize prevention (including social determinants, contribute to seamless care, build community capacity and demonstrate collaboration.
Budget requirements:
Equipment purchases can not exceed $5,000 and computers no more than $2,500 with total equipment not to exceed 50% of funds requested.
Indirect costs applied to personnel and direct operational expenses cannot exceed 15%.
Indirect costs are expenses that support an organization's overall operations but aren't directly tied to a specific project or program. Examples include rent, utilities, telecommunications, office supplies, insurance, and salaries/benefits for individuals supporting the organization’s general operations (i.e., accounting, clerical, finance, managerial).
Ineligible proposals:
Any proposals with unrestricted general operating support, event sponsorships and activities less than 9 months in duration. Funding for capital projects, lobbying or political campaigns.
