Dental Hygiene Program

At California Hospital, we believe in a full-spectrum approach to health. That's why we partner with local dentists to provide free dental services to our patients in need.

Periodontal (gum) diseases range from simple gum inflammation to serious disease that results in major damage to the soft tissue and bone that support the teeth.  In the worst case, teeth are lost.  Patients with diabetes are at greater risk for periodontal disease and experience a more destructive, aggressive form of the disease; in fact, periodontal disease is often called the sixth complication of diabetes.

The Community Dental Partnership is a collaboration of the Southside Coalition of Community Health Centers and Eisner Health.  Uninsured adults with medication-dependent diabetes whose medical home is one of the clinics of the Southside Coalition are eligible for this free program.

The following  services are provided under this program:

  • Oral Hygiene class in English or Spanish – mandatory prior to receiving first dental appointment
  • Basic dental services if not offered by medical home
  • Periodontal services and follow-up if deemed necessary by referring dentist from medical home or dentist at Eisner Health (whoever provides basic dental services)
  • Diabetes Education Empowerment Program if diabetes education is not offered by medical home
