Dignity Health’s Central Coast Service Area Provides $308,127 in Grant Funding to Community Non-Profit Organizations

The grants will help provide critical health and human services to San Luis Obispo County and Santa Barbara County residents

SAN LUIS OBISPO, CA – February 1, 2019 – Dignity Health Central Coast, which includes Marian Regional Medical Center (MRMC), Arroyo Grande Community Hospital (AGCH) and French Hospital Medical Center (FHMC), is proud to announce it has awarded $308,127 in grant funding to five unique community non-profit partnerships in San Luis Obispo and Santa Barbara County. Representatives from each organization were invited to a breakfast on January 25, 2019 where they were presented with checks to help aid in the critical health and human services they provide to county residents. 

Grants are awarded through a Community Grants Program created by the Central Coast Service Area’s parent company Dignity Health. The intent of the annual community grant program is to help non-profit organizations with an interest in building healthier communities by improving health and living conditions. Criteria for this funding cycle were based on the following priority areas identified in the hospitals Community Benefits Report and Plan and Community Health Needs Assessment: Access to Healthcare including Mental Health, Education, Homelessness/Housing, Cardiovascular Disease & Stroke, and Cancer Prevention & Screenings.

This year, the Dignity Health Community Grants were awarded to the following local Accountable Care Communities: 


Recuperative Care Program  

Program Description: This accountable care community will provide respite care to homeless patients with  24/7 staff monitoring by a RN,  which will increase access to primary care, mental health counseling, and case management resulting in a integrating medical and behavioral treatment in a continuum of care.

Community Resources for the Underserved in Los Osos 

Program Description: This accountable care community will improve the health of participants by providing healthy meals, hygiene supplies, and shelter during winter months, and socialization. This project will also improve access and utilization of community support services for basic needs, behavioral health, health education and access to treatment (medications) for chronic health conditions such as diabetes and heart disease.

Central Coast Healthcare and Housing 

Project Description: This accountable care community will continue offering a primary health clinic on site which will increase access to cancer screening, primary care, pharmaceutical and social services for low-income, uninsured, and under-insured in North County San Luis Obispo residents. In the Guadalupe area it will offer health education and access to treatment (medications) for chronic health conditions such as diabetes and heart disease.

Accountable Care Community (ACC): Peoples’ Self Help Housing, SLO Noor Foundation, Alliance for Pharmaceutical Access Inc. (APA), and Dignity Health Northern Central Coast Service Area Hospitals


Collaborative Services for Southern SLO Co Homeless - Medically Fragile and At-Risk

Program Description: This accountable care community will strengthening the local continuum of resources targeted to those who are homeless or facing homeless in southern San Luis Obispo County, especially those who are medically fragile or having high needs. Efforts will improve care transitions from hospital bed to respite care, improve awareness of and access to existing health and social services, provide substance abuse treatment and improve the nutrition of clients served. 

Fiscal Agent: 5 Cities Homeless Coalition  

Accountable Care Community (ACC): 5 Cities Homeless Coalition, Central Coast Freedom Network, Shower the People, and Arroyo Grande Community Hospital


Learning, Training & Counseling Center Work Program

Program Description: This accountable care community will provide work training, employment assistance, and shelter and housing assistance to homeless residents in Santa Maria. The objective of the program is to provide homeless individuals in the community with work experience and skills to increase confidence and self-esteem and to help become contributing members in the community. 

Fiscal Agent: The Salvation Army 

Accountable Care Community (ACC): The Salvation Army, Miranda’s Landscaping, Good Samaritan Shelter, Workforce Resource Center, United Way, and Marian Regional Medical Center
